Required Letter for Soliciting External Evaluations Using this template is required; however, additional information can be added.
Required Annotated Listing of External Evaluator is required.
When a Department Head, Dean or Provost receive unsolicited letters or emails pertaining to a case, these should not be included in the dossier, but should be retained in the faculty member’s personnel file.
